The goods and services shown inside are those we can provide to our customers. You may choose only those items you desire; however, any funeral arrangements you select will include a charge for our services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

This list does not include prices for certain items that you may ask us to buy for you, such as cemetery services, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected.

For the family's convenience, we have put together several different types of services and prices for each service. For a total breakdown of the charges, please refer to the itemized listing of prices on the next pages.

PRIOR TO DRAFTING ANY CONTACT FOR GOODS OR SERVICES, THE RESPONSIBLE PARTY, OR THE DECEDENT'S SURVIVOR WHO IS HANDLING THE FUNERAL ARRANGEMENTS IS ENTITLED TO RECEIVE A COPY OF ANY PRENEED AGREEMENT IN THE POSSESSION OF THE FUNERAL ESTABLISHMENT THAT HAS BEEN SIGNED AND PAID FOR, IN FULL OR IN PART, BY OR ON BEHALF OF THE DECEDENT.

ALTERNATIVE PACKAGE SELECTION

OUR DISCOUNTED PACKAGE SELECTIONS ARE OFFERED TO THOSE FAMILIES WHO PURCHASE THECASKET/URN FROM FEHRMAN MORTUARY. FAMILIES PURCHASING CASKETS/URNS FROM OUTSIDE AGENCIES, OTHER THAN FEHRMAN MORTUARY MAY SELECT SERVICES ONLY FROM THE ITEMIZED SECTION OF THE GENERAL PRICE LIST. FEHRMAN MORTUARY WILL CASH ADVANCE ONLY STATE DISPOSITION FEES/DEATH CERTIFICATES (NOT TO EXCEED $100.00)

Our package selections are designed to simplify the arrangement process and to assure the family that all desired services will be provided without additional cost and to offer a significant monetary savings compared to the itemized price list. Although services may be selected the from the itemized portion of the General Price List, the above list of commonly selected package services may be selected from the itemized portion of the General Price List, the above list of commonly selected package services may help you to select the appropriate arrangements. The above packages include only the items listed. Additional merchandise you select and the cash advance items are not included. The non-use of goods and services contained within any of the alternative packageselections will not result in a reduction in the price of the alternative package selected. You may elect to purchase specific goods and services on an itemized basis.

OUR POLICY FOR PAYMENT OF OUR SERVICES ARE AS FOLLOWS: IF A FAMILY WANTS TO PAY BY INSURANCE-WE WILL ACCEPT ALL INSURANCE THAT WILL ACCEPT AN ASSIGNMENT TO FEHRMAN MORTUARY AND CREMATORY

A SERVICE CHARGE OF 4% WILL BE CHARGED ON ALL CASH ADVANCES PAID WITH A CREDIT CARD
